🩸 New and classic anticoagulants. What are they and what is the difference?

📌 Anticoagulants are medications designed to reduce the blood's ability to clot🩸, lowering it (not eliminating it) to levels that prevent diseases or situations predisposing to clot formation. The most common conditions include atrial fibrillation, deep vein thrombosis (DVT), or pulmonary embolism.

What are the oral anticoagulant drugs used? The older or classic ones are Warfarin and Acenocoumarol, very safe and effective medications, but they must be monitored by an expert every MONTH with a coagulation test known as INR. Their significant variability requires strict monthly dose adjustments. Too much effect can be dangerous, as it increases the risk of bleeding🩸🩸, and too little effect doesn't fulfill its therapeutic function.

On the other hand, in recent years, new anticoagulants (Rivaroxaban, Apixaban, or Dabigatran) known as DOACs or direct oral anticoagulants have emerged. Although they are more expensive, they have greater clinical benefit and do not require coagulation monitoring. However, they do require renal function monitoring every few months.

A very important point to note is that the new anticoagulants cannot be interchangeably used with the old ones for all diseases or situations. In some cases, Warfarin or Acenocoumarol are the only viable options, such as in mechanical heart valve prostheses.
